'Humans of New York' has partnered with the United Nations Refugee Agency to deliver the personal stories of those fleeing North Africa.

Brandon Stanton, the photographer behind the initiative, has taken images of refugees and spoken with them to convey their story in emotional posts on Facebook.

"Together, these migrants are part of one of the largest population movements in modern history," he wrote, "But their stories are composed of unique and singular tragedies."

The page has over 15 million followers.

A record 522,134 refugees and migrants have landed in Europe by sea so far this year, the International Organisation for Migration said this week.
