"I don't like it when people say I was born a boy," begins 13-year-old transgender teen, Jazz, in the brave and heartwarming video above.
"I'd prefer people say I was born a girl trapped in a boy's body."
Jazz first touched audiences back in 2007, when she was just 6 years old. Since she has been a strong role model and active campaigner for transgender young people, and is the co-founder of the Transkids Purple Rainbow Foundation.
She adds: "I want to pass my confidence and pride of being transgender to others... I want everyone to understand that they are special and be proud of who they are."
Last year, Jazz told HuffPost Live that about her dreams of becoming a mother in the future.
"I want to be the mother of my own child, but I know if I adopt, I'll also be a great mother anyways and I'll give them all the love I have inside," Jazz said. "I know there will be someone who I can give my love to."
What an inspirational young woman.
